If you think it's RAM, then I'd recommend squeezing the chips. Sometimes you can find a bad connection that way.

Hmm, maybe try underclocking to 143 MHz or lower and see if you can tease out another clue. You have to use the VSA100 Overclocker by KoolSmoky with the Voodoo5 to ensure that both chips receive the clock change. https://www.3dfxzone.it/enboard/topic.asp?TOPIC_ID=9203

It doesn't need a fan if you're not overclocking, a heatsink would be enough. I'd also go adhesive tape as you've listed for ease of removal and because it's a low temperature scenario. Go cheap, no need to spend a lot unless you want something nice to look at.

If you want to know for sure, take out the CPU and look at the back side for a burn mark. Just buy a cheap XP to test with, like $20 shipped and make sure it works with your board before you buy it. Buy the expensive one later.

I'm going to guess that your setup pulls 120W max and I think s370 is a 12V CPU so a 200W - 240W Lite-On should be fine. I'd say Lite-On is a hit and miss brand.
