I think I have a slightly older version of that card: IMG_20240423_195320.jpg IMG_20240423_195332.jpg It says "MediaMagic ISP-16" on the PCB and indeed has a MAD16 (manufactured by Mediachips???). It also seems to have jumpers for configuration so I'm not sure that it actually the same card as yours …

If you're in Win9x dos box, which MPU port are you using? Because this card actually provides two MPU devices, one is the Dream, the other one is the crappy semi-software synth the Maestro chip provides.

Savage3D is not a particularly fast card. I also get ~30fps in Quake 2 on a K6-III/450. Furthermore, I get these numbers even on an 800MHz Athlon system. Seems like this is the cards' performance after all...

Had the same issues on the same type of board some time ago. In the end it turned out that the PSU was faulty and the +5V rail was unstable and too low. The crash on the HW monitor screen might be a BIOS bug, my board behaves in the same way.

There is a small fuse next to the keyboard connector, right in the corner of the PCB. It looks like a green or yellow resistor and is marked as "F1" on the PCB. Maybe it has blown, this would prevent the keyboard from getting any power. You might want to check that fuse for continuity.
